Personal Accidental

1. Coverage –
As a valued Customer registered under Group Master Policy # 0236353750 you will be covered for –

a) Accidental Death, Permanent total disability
Sum Insured: As per Plan Opted

b) Accident hospitalization medical expenses (Admissible if claim is payable under accidental death and/or for permanent total
disability).
Sum Insured: Up to 10% of CSI or 20% of admissible claim amount or actual whichever is lower, with max up to 15,000/- per
person.

c) Repatriation of remains (funeral benefit)
Sum Insured: Maximum up to Rs. 3,000/-

d) Ambulance charges
Sum Insured: Max up to Rs. 1,000/- per person
The personal accident cover is offered to registered individual customers who have opted for Value added services from
WideCare. Maximum sum insured payable for death and permanent total disability per registration is as per plan opted. All admissible claims are payable as per terms & conditions of the master policy.Any act of terrorism is covered.Age limit of the Insured: 18 years to 70 years. Multiple enrolments/registrations of customer for the benefit are allowed up to maximum of 3 registrations, and not exceeding Rs. 9,00,000/- in cumulative. By default, the death claim will be settled to the legal heir.
In case if the registered individual customer opts out of the Value-added services from WideCare, policy will lapse.


2. Claim Procedure –
Upon the happening of any event which may give rise to claim under this policy, written notice with full particulars must be given to the company immediately. Complete documentation and satisfactory proof shall be furnished for all matter upon which a claim is
based. Additional documents/ information may be called for and/or verification may be carried ou where felt necessary.

6. Attested copy of Identity proof of insured and nominee

7. Attested copy of KYC documents of nominee

Medical expenses

3. Original Disability Certificate from the Competent Authorized Doctor with the % of disability

4. Attested copy of complete medical records including Investigation/ Lab reports (X-Ray, MRI etc.)

5. Attested copy of Admission/ discharge card

6. Attested copy of FIR & Panchanama

7. Photograph of injured area

8. Attested copy of Pan card & residential proof

9. Cancelled cheque & Latest Passport size photograph

Permanent Total Disablement Claims

1. Claim form

2. Attending Doctor’s Report

3. Original Disability Certificate from the Doctor

4. Attested copy of Investigation/ Lab reports (x-ray etc.) along with copy of Admission/discharge card, if hospitalized

5. Attested copy of FIR / Police Inquest report, if any

7. Attested copy of KYC documents of insure

8. Latest Passport size photo & cancelled cheque of nominee bank account
